Buffalo Valley, Inc. has provided affordable alcohol and substance abuse treatment in Hohenwald, Tennessee for more than 30 years. Incorporated in 1979 as a small community-based program, the organization has grown into a comprehensive nonprofit that combines clinical treatment with case management, housing assistance and workforce development – a continuum aimed at the very low-income, disabled and homeless residents who carry the heaviest burden of addiction in the region.
The clinical continuum includes medically monitored detox, residential rehab, a partial hospital program and intensive outpatient care, along with specialized services for co-occurring disorders, veterans and clients living with military-related PTSD (PTSD-M). Treatment is delivered by counseling staff, with psychosocial assessment guiding each individual’s care plan. The organization holds Joint Commission accreditation and reports roughly 3,500 patients served annually, making it the largest TennCare-funded alcohol and drug treatment provider in the state.
Recovery at Buffalo Valley is intentionally practical. Over the past decade and a half, the organization has added job training, job placement assistance, GED preparation, life skills training and case management to its clinical core, recognizing that stable employment and identity are essential to sustained sobriety. A separate Valley Housing division provides affordable housing across middle Tennessee, supporting graduates and other low-income residents with rental options that reduce relapse risk tied to homelessness.
Veterans are a particular focus, with VA Housing and PTSD-M programming integrated into the broader continuum. Executive Director Jerry T. Risner, who has led treatment at Buffalo Valley for more than 25 years, has reported that roughly half of patients remain sober a year after discharge – a notable outcome for a population that often arrives with limited resources.
