About CODAC Pawtucket Avenue
CODAC Pawtucket Avenue is located at 600 Pawtucket Avenue in Pawtucket, Rhode Island, and is one of eight CODAC Behavioral Healthcare locations statewide. Founded in 1977, CODAC is one of Rhode Island’s most established nonprofit providers of outpatient behavioral healthcare, and the Pawtucket Avenue site offers the full range of services that have made the organization a cornerstone of the state’s behavioral health system.
Clinically, the site focuses on outpatient treatment for substance use disorders and co-occurring mental health conditions. Cognitive Behavioral Therapy and Motivational Interviewing anchor the therapeutic model, supported by individual and group counseling. CODAC’s Substance Abuse Treatment Team (SATT) is coordinated by Clinical Director Raymond A. Lorraine, MA, ADS, CADC, LCDP, TTS. Specialty programs include the HOPE Initiative — Rhode Island’s coordinated response to the opioid crisis — directed by Dennis Leahey, plus gambling disorder treatment and tobacco cessation services. Amanda Martins serves as Administrator.
The Pawtucket Avenue location offers more than clinical services. CODAC hosts community meeting and group rooms available to community partners, runs trainings and workshops for professionals across Rhode Island, and provides 24-hour access to care, seven days a week. The newly opened Royal Little Drive location in Providence — part of the same CODAC network — adds fully integrated primary care, dentistry, and additional services that complement the outpatient behavioral health programs offered in Pawtucket.

As part of its statewide network, CODAC also operates correctional facility programs and mobile medical units that reach underserved communities, ensuring that clients across Rhode Island have access to evidence-based behavioral health care regardless of where they live or work.
