The Commitment House operates a residential and outpatient addiction treatment program at 109 Parkway Drive in Elizabethtown, with affiliated locations in Lexington and Louisville extending the reach across Kentucky and into Indiana. The program is built around a deliberate step-down model that takes clients from medical withdrawal management all the way through long-term sober living, with consistent clinical staff at each phase.
The continuum begins with detox combined with residential treatment. The medical team provides withdrawal management with comfort medications, and clients can be admitted with PICC lines in place — an important distinction for patients leaving the hospital with active IV antibiotic regimens for endocarditis, abscesses, or other complications of injection drug use. Residential treatment runs 28-30 days and focuses on education, stabilization, and the development of recovery tools.
From residential, clients step down through a four-week Residential Transitional phase with twenty-four-hour support and clinical groups five days a week, then into Partial Hospitalization at five hours per day in a sober-living setting, followed by Intensive Outpatient at three hours per day three days per week for eight weeks, and finally a weekly Outpatient phase focused on discharge planning and independent living. Throughout, clients live in supervised sober housing, attend 12-step meetings, and receive case management and peer support.
The Commitment House is CARF-accredited and accepts most major insurance plans including Medicaid, state insurance, TRICARE, and private commercial plans, with self-pay options for clients without coverage.
