Cornell Scott-Hill Health Center’s South Central Rehabilitation Center (SCRC) is the addiction treatment arm of one of southern Connecticut’s largest Federally Qualified Health Centers. Headquartered at 232 Cedar Street in New Haven, Cornell Scott-Hill Health Center (CSHHC) serves approximately 57,000 patients each year across 38 locations, integrating addiction treatment with primary care, pediatrics, women’s health, dental, pharmacy, and behavioral health services in a Patient-Centered Medical Home (PCMH) model.
The rehabilitation program offers detox treatment, methadone maintenance, and broader addiction services for adults working through drug and alcohol use disorders. As the largest provider of outpatient behavioral healthcare in southern Connecticut, CSHHC’s adult behavioral health team supports patients with co-occurring mental health conditions in coordination with their addiction care. This integration helps reduce the gaps that often exist between behavioral health and medical care, particularly for patients with complex needs.
CSHHC is accredited by The Joint Commission and has been recognized by the Health Resources and Services Administration (HRSA) as a Health Center Quality Leader, a High Value Care center, and a leader in Advancing Health Information Technology for Quality. The organization holds Patient-Centered Medical Home recognition and has been recognized as a HRSA Health Disparities Reducer, underscoring its commitment to equitable care.
Services are available to patients regardless of insurance status, with eligibility based on income and family size, and CSHHC uses a sliding fee scale.
