Transformations Care operates a residential and outpatient addiction treatment campus in Gardena, California, in the South Bay area of Los Angeles County. The program spans an unusually broad continuum, offering medical detox, residential inpatient treatment, partial hospitalization (PHP), intensive outpatient (IOP), and traditional outpatient programming for substance use disorders, plus parallel mental-health-only PHP, IOP, and outpatient tracks for clients whose primary presenting concern is psychiatric rather than substance-related.
The program specializes in personalized treatment plans built around each client’s clinical needs, substance history, and life circumstances. Substances treated include alcohol, methamphetamine, opioids, heroin, prescription drugs and painkillers, and cocaine, with medication-assisted treatment options including Suboxone and methadone available for opioid use disorder. Clinical Director Charleen Meyer (LMFT, ATR) brings both licensed marriage and family therapy and registered art therapy credentials to a clinical team that integrates CBT, individual and group therapy, family counseling, and an Inclusive Indigenous Wellness approach for clients drawn to culturally rooted recovery practices.
Transformations Care places a deliberate emphasis on family integration, recognizing that addiction rarely sits inside one person in isolation. Family integration services, structured family counseling, and an alumni aftercare program support both the client and their family system through and beyond treatment. The facility caters to a wide population mix including veterans, pregnant women, young adults, older adults, court-referred clients, and clients with co-occurring HIV/AIDS or trauma histories.
Transformations Care is accredited by The Joint Commission and is in-network with most major PPO plans including Aetna and Blue Shield of California. The program accepts TRICARE for military families, self-pay, and sliding-scale arrangements, but is explicit that it does not accept Medi-Cal or Medicare at this time. Insurance verification requires only an insurance card, full legal name, date of birth, and a callback number.
